Step 1 - Click on the Browse Button to locate the file on your hard drive and click choose file
Step 2 - You will see your file name next to the Browse Button but it has not been attached to the form yet, you must now click the Add File Button to attach that file to this submission form. To add up to file 5 files for a total size limit of 10MB repeat Steps 1 & 2. If you need to Remove a file after it has been added just click the Remove Link next to the file name.
Step 3 - Complete the rest of the online form and click the Submit Button
